The only CETP inhibitor still in active development, and the first to be built on an LDL-lowering claim rather than an HDL-raising one. A once-daily 10 mg oral tablet, taken on top of maximally tolerated statin therapy. BROADWAY, BROOKLYN and TANDEM read out in 2024 and 2025; the CHMP recommended marketing authorisation on 24 July 2026 for Ubeslo (monotherapy) and Evlarco (fixed-dose combination with ezetimibe). The cardiovascular outcomes trial, PREVAIL, has not read out.

Mechanism. Obicetrapib binds CETP and blocks the transfer of cholesteryl esters from HDL to apoB-containing lipoproteins. The consequence that matters clinically is not the HDL rise but the fall in LDL-C and apoB, which is where the entire development programme is now aimed.

What is proven. In the phase 3 programme, obicetrapib lowered LDL-C by up to about 40% as monotherapy versus placebo, and by roughly 50% in fixed-dose combination with ezetimibe, in patients already on maximally tolerated lipid-lowering therapy, with tolerability comparable to placebo. Those three trials, BROADWAY (n=2,530), BROOKLYN (n=354, heterozygous familial hypercholesterolaemia) and TANDEM (n=407), are the basis of the European filing.

What is not proven. No CETP inhibitor has ever shown a cardiovascular outcome benefit in a completed trial that was allowed to run to its endpoint. PREVAIL (NCT05202509, n=9,541) is the trial that will answer that question for obicetrapib; its primary completion date is November 2026. Until it reads out, obicetrapib is an LDL-lowering drug with a regulatory file, not a proven preventive therapy.

Why this one might be different. The three drugs that failed did so for reasons that are, at least on paper, specific to them: an off-target pressor effect (torcetrapib), a weak effect on apoB (dalcetrapib), and futility against a background of intensive statin therapy (evacetrapib). Anacetrapib, the one that did reduce events, was shelved for reasons of adipose accumulation and commercial judgement rather than efficacy. Obicetrapib is more potent per milligram on LDL-C than any of them, and the genetic evidence says apoB lowering is what CETP inhibition should be judged on.
