PREVAIL
Status Active, not recruiting; primary completion November 2026
The cardiovascular outcomes trial the whole class has been waiting for: 9,541 patients at high risk, randomised to obicetrapib 10 mg daily or placebo on top of maximally tolerated lipid-lowering therapy. Started February 2022, primary completion November 2026. If PREVAIL is positive, CETP inhibition becomes a therapy; if it is neutral, the class is finished as a preventive strategy whatever the LDL-C numbers say.
PREVAIL is event-driven and the only trial on this site whose result can change practice on its own. Obicetrapib already has a positive CHMP opinion on its LDL-lowering file, so the unusual situation in late 2026 is a drug that may reach the market in Europe before its outcomes trial reports.
What to watch for when it reads out: the absolute event reduction against the achieved apoB difference, whether the benefit tracks apoB the way the Mendelian randomisation evidence predicts, and the blood pressure record over the full follow-up.
